Dear Deans and Research Advisers,

Please note the following reminders for your Students’ Initial Submission and Resubmission of Research Protocol Requirements to the IERB:

INITIAL SUBMISSION:

1.    FORM 13 must have the Study Protocol (Chapters 1-3) Attachment and FORM 14 must have the Informed Consent Attachment. These two forms with their respective attachments must be in separate files.

2.    Please make sure that the study protocol and the informed consent are properly numbered in terms of the pages and the paragraphs. These numbers  must coincide with the numbers referred to in FORMS 13 and 14.

3.    The Diagrammatic Workflow is not a Gantt Chart. The workflow summarizes the methodology to be undertaken in a diagrammatic manner with lines/arrows and boxes as representation.

4.    For the FORM 12, for undergraduates and GS (subject requirement) Section II will be signed by their research adviser and for GS students' Masters Thesis and Dissertation, Section II will be signed by their research adviser and panel chair. Section III will be signed by their respective deans.

5.    For Graduate Students with Thesis/Dissertation, they must submit their research proposal after getting an approval from their panelists NOT before.

6.    All files must be submitted separately from each other

RESUBMISSION: All changes made on their protocol must be highlighted for easier review.
